Below is a list of recently released U.S. Department of Health and Human Services (HHS) Funding Opportunity Announcements for programs administered by Center for Disease Control and Prevention's (CDC) National Center for Chronic Disease Prevention and Health Promotion.

PPHF 2012: REACH: Racial and Ethnic Approaches to Community Health financed solely by 2012 Prevention and Public Health Funds

Letter of Intent (LOI) Deadline Date: July 11, 2012; by 5:00 pm U.S. Eastern Daylight Savings Time

Application Deadline Date: August 7, 2012; 11:59 pm U.S. Eastern Daylight Savings Time.

Purpose: This funding opportunity announcement (FOA) is designed to fund national or multi-state organizations to:

· Fund, manage, support, and monitor sub-recipients to address health disparities and implement evidence- and practice-based strategies that reduce health disparities for intervention population(s) experiencing high burden of disease or risk factors. The intervention population is the population within the selected geographic area that applicants will reach with proposed program activities.

· Contribute to the development and implementation of a community health action plan to address chronic disease health disparities.

 


 

PPHF 2012: REACH: Racial and Ethnic Approaches to Community Health: Obesity and Hypertension Demonstration Projects financed solely by 2012 Prevention and Public Health Funds

Application Deadline Date: August 7, 2012; 11:59 pm U.S. Eastern Daylight Savings Time.

Purpose: Support the implementation arm of demonstration projects focused on reducing health disparities. It builds upon previous Racial and Ethnic Approaches to Community Health (REACH) programs ( www.cdc.gov/reach) and other health equity-related community health programs and serves as a focused complement to programs working to make population-wide, place-based improvements. Recipients funded through this FOA will develop and implement replicable and scalable strategies to assure that population-wide policy, systems, and environmental (PSE) improvements to reduce obesity and hypertension also decrease health disparities in these morbid outcomes in their jurisdiction. In addition, recipients will work in close collaboration with a national evaluation team (funded through a separate contract) who will assist in assessing the impact and effectiveness of the strategies implemented through this FOA. Together, the implementation and evaluation of strategies designed to enhance benefits and eliminate barriers to population-wide and targeted PSE improvements will help build evidence and expand the reach of strategies to address health disparities. Thus, all recipients are required to fully participate in evaluation activities, including collection of local data, which will be supported by CDC and evaluation partners tasked with rigorously evaluating the implementation efforts. Recipients will then disseminate their strategies and mentor other communities, as appropriate.

 


 

Partner Support for Heart Disease and Stroke Prevention

Purpose: To provide funds for organizations with national reach (the capacity to guide the Nation in taking action) to support implementation of national, state, and local level strategies that align with select recommendations from the Update to A Public Health Action Plan to Prevent Heart Disease and Stroke and Million Hearts™ initiative.This work includes promoting effective communication, leadership, and action to support prevention and control of heart disease, stroke, and risk factors such as high blood pressure, high LDL cholesterol, and high sodium intake. Grantees will be asked to help broaden, strengthen, improve, or sustain opportunities for collaboration and partnership at national, state, and local levels across the public and private sectors.

 


 

Obesity Prevention Programs-Early Care and Education (ECE) Statewide Collaboratives to Improve Nutrition, Breastfeeding Support, Physical Activity and Screen Time Practices for Obesity Prevention in Young Children financed solely by 2012 Prevention and Public Health Funds

Application Deadline Date: July 17, 2012, 11:59 pm Eastern Standard Time

Purpose: To improve the quality of obesity prevention practices in ECE facilities by providing opportunities to ensure equitable access to ECE services that meet the national Preventing Childhood Obesity (PCO) (2 nd Ed.) standard components related to nutrition, breastfeeding support, physical activity, and screen time, all of which are key areas for obesity prevention.

 


 

PPHF 2012: Breast and Cervical Cancer Screening Opportunities for States, Tribes and Territories Financed solely by 2012 Prevention and Public Health Funds

Application: July 18, 2012 at 5:00 p.m. U.S. Eastern Standard Time

Purpose: To increase the number of women screened for breast and cervical cancer screenings by approximately 50,000 to provide high quality screening with tracking and follow-up including patient navigation to low income, uninsured and under-insured women. This FOA will leverage the existing infrastructure and organized delivery system of the NBCCEDP and all requirements of this program related to eligibility and screening are required; these requirements of the NBCCEDP are outlined in PL 101-354 and DP12-1205.
