CDC National Implementation & Dissemination for Chronic Disease Prevention Grant

To prevent chronic diseases like diabetes, cardiovascular disease, arthritis and many forms of cancer, by helping communities boost physical activity, nutritious eating and other healthy ways of living, the Centers for Disease Control and Prevention (CDC) funded the National Implementation & Dissemination for Chronic Disease Prevention Grant also known as Partnering4Health

 

The grant was awarded to a collaboration of five national organizations:

American Heart Association, American Planning Association, and National WIC Association

Directors of Health Promotion and Education, and the Society for Public Health Education,

 

Through Partnering4Health, 94 local health coalitions were funded to improve their community’s health through:

v  improved access to healthy foods and beverages;

v  more access to physical activity opportunities;

v  more smoke-free environments;

v  increased access to clinical preventive services;

v  coordinate national and regional external communications of the resulting public health messaging; and

v  widely disseminate stories of changed communities.

 

How Did They Do That?  Watch the recorded Partnering4Health White Paper Webinar to hear the summative highlights of the overall successes and lessons learned from the partnership of these five national organizations.  Additionally, the webinar shares the grant’s sustainability plan and a microsite featuring the many resources that were produced in result of the grant.
