Individual Voting (for 2014: $400)
Voting Membership shall be open to the state or territorial level director of health promotion or public health education, IHS Area Health Education Director, or the equivalent.

Organizational (Voting + 4)(for 2014: $600)
An Organizational Membership entitles a state health agency to one Voting Membership and four (4) Associate Memberships.

Associate Membership (for 2014: $75)
Associate Membership is open to any individual health professional employed in the area of public health education/health promotion supporting the purposes of the Association.

Student Membership (for 2014: $50) - New

Student membership shall be open any individual who is a student enrolled in a degree program at an institution of higher education supporting the purposes of the Association.

Emeritus Membership (for 2014: $50) - New
Emeritus membership shall be open to former Voting Members who have retired from state health department service and from any employment in public health, health promotion, health education, or health equity.

Partner Membership (for 2014: $300) - New
Partner membership shall be open to academic, government, and not-for-profit that supports the purposes of the Association. Each Partner membership entitles the organization to five (5) Associate memberships.

Corporate Membership (for 2014 $2000) - New
Corporate Memberships shall be open to for-profit business and organizations that support the purposes of the Association. Corporate membership entitles the corporation to unlimited Associate memberships.

Memberships are on an annual calendar year basis. Memberships can be transferred to another individual within the same organization at any time. Dues purchased from October 15 forward will extend through December of the following year.

Membership Cancellation: Membership cancellations received within 30 days of registration may be eligible to receive a full refund. Cancellations received after the stated deadline will not be eligible for a refund. Cancellations will be accepted via phone (202-659-2230), fax (202-478-0884),ore-mail, and must be received by the stated cancellation deadline directly from the named member or the individual who paid for the membership. Refunds will be credited back to the original credit card used for payment or a mailed check for check payments.
