Description

This webinar will describe how community health organizations can partner with health care systems and providers to increase utilization of community resources and services for chronic disease risk reduction and management. Presenters will share strategies for creating and strengthening bi-directional referral systems, including how to work with state health departments. Awardees will share “what works” and lessons learned when initiating and developing these partnerships.

 

Learning Objectives

By the end of this webinar, awardees should be able to:

·        Describe the role of state and local health departments in increasing bi-directional referrals

·        Identify at least two lessons learned for initiating and strengthening relationships with health care systems and providers

·        Identify at least one resource for working with health care systems and providers on bi-directional referrals
