CDC’s Division of Community Health (DCH) is pleased to announce the following webinar in the TACTIC series:

Making the Link Part I- Increasing Access to Community Programs for Chronic Disease Prevention and Management

This webinar will provide strategies on how community health organizations can work with key stakeholders to help increase access to clinical and community preventive services, including chronic disease management. Presenters will address: 1) insurance coverage for clinical and community services; 2) access barriers and 3) the key role you can play when working with insurers, employers, health care providers, and others.

Learning Objectives
By the end of this webinar, awardees should be able to:
• Describe common access barriers to community chronic disease management programs
• Recognize what services are and are not typically covered for community preventive services
• Identify strategies for working with key stakeholders to help improve access to services
