What: Clinical-Community linkage (CCL) interventions connect clinical enterprises with community programs focused on healthy eating, active living, and/or chronic disease management to benefit patients at risk for or diagnosed with chronic conditions. Patient referrals to community resources are one mechanism for making this linkage between clinics and communities. This webinar will provide an overview of CCL referral interventions and describe the status of the evidence-base. Awardees will also learn about how to use electronic medical records (EMR) to generate data to assess actual use or conduct outcome evaluations of referral interventions. Lastly, the webinar will highlight the work of two awardees who will share their experiences with EMR data for actual use assessment and outcome evaluation.
