Award Nomination Guidelines

You may submit a nomination on-line and also encourage other health partners to submit nominations. This is an excellent opportunity to highlight both statewide and national efforts to promote individual and community health.

There are five award categories. Three awards (Health Promotion Medal of Excellence, State/Community Collaboration Award, the Health Promotion and Education Advocacy Award) provide for broader participation. Two of the awards are specifically for DHPE members (Leadership Award, Evans/Muneoka Award). Award nominations may be submitted by anyone.

The following guidelines should be used for all nominations:

  • Use the on-line form to nominate your candidate.
  • Use a separate form for each nominee and each award.
  • Describe your nominee's contributions to health promotion giving specific examples.
  • Click on "Add New Award" to submit the nomination.
  • Previous award recipients are eligible for those categories in which they have not received an award.
  • Your nominations must be submitted by May 23, 2008
