Leadership Award:

This award honors the State, Territorial or Indian Health Service Area Director of Health Education/Health Promotion or an DHPE Associate Member whose leadership has resulted in a demonstrable coalescence of state and local level constituencies toward the goal of expanding health promotion/health education activities in the state. Evidence of changes in risk prevalence, health status or influence on national level should be taken into account.

Final Check - did the candidate that you are nominating meet the following minimum criteria for the award nomination?

  • Is the candidate a State, Territorial, or Indian Health Service Area Director of Health Education/Health Promotion or an DHPE Associate Member?
  • Did the candidate demonstrate coalescence of state and local level constituencies toward the goal of expanding health education/health promotion activities in their state?
  • If possible, provide additional evidence regarding risk prevalence, health status or influence on national level.

Evans/Muneoka Award

This award honors a State, Territorial or Indian Health Service Area Director of Health Education/Health Promotion or an DHPE Associate Member who exemplifies consistent and extraordinary mentoring and professional support to health promotion and health education staff; state, local and national colleagues and/or other members. The award is meant to honor the memory of members, Karen Evans and Laurel Muneoka, who lived their professional lives as professionals who went out of their way to provide special attention and mentoring to others they worked with, regardless of status or setting.

Health Promotion Medal of Excellence:

This award honors the achievement of a "state-level" health education/health promotion director or health educator whose work has made a contribution to the state's capacity to apply science to the field of health education and health promotion.

  • Is the candidate a state level health education/health promotion director or health educator?
  • Did the candidate's work make a contribution to the state's capacity to apply science to the field of health education/health promotion?

State/Community Collaboration Award

This award honors statewide or community group efforts that have resulted in evidence of coalescence between state or community level constituents in demonstrating innovative health education and health promotion practices at the state or community level.

Final Check - did the candidate that you are nominating meet the following minimum criteria for the award nomination?

  • Is the nomination for a statewide or community group effort?
  • Did the nomination result in the coalescence between either state or community level constituents?
  • Did the nomination demonstrate innovative health education/health promotion practices at the state or community level?

Health Promotion and Education Advocacy Award

This award honors a person for his/her advocacy to further health education as a profession and/or who has served as a champion in promoting the science of health promotion and disease prevention at the state or national level. Nominations are also being accepted under this category for policy makers who promote prevention efforts in public health.

Final Check - did the candidate that you are nominating meet either of the following minimum criteria for the award nomination?

  • Is the nomination for an individual person?
  • Did the candidate serve as a champion in promoting the science of health promotion and disease prevention at the state or national level?

Or

  • Is the candidate a policy maker who promoted prevention efforts in public health?
  • Does the candidate demonstrate his/her advocacy to further health education as a profession?
